Central City clinics have on-site Medi-Cal Eligibility, you can find a CCCHC clinic near you from the list of nearest Central City health centers or call the Call Center for more information (866) 332-9550.

What you will need:

  • Proof of household income
  • California ID
  • US Passport, Legal Resident card, Certificate of citizenship or naturalization document.
  • Proof of address
  • Household social security numbers
