Central City clinics have on-site Medi-Cal Eligibility, you can find a CCCHC clinic near you from the list of nearest Central City health centers or call the Call Center for more information (866) 332-9550.
What you will need:
- Proof of household income
- California ID
- US Passport, Legal Resident card, Certificate of citizenship or naturalization document.
- Proof of address
- Household social security numbers
For those who don’t qualify for Medi-Cal there are other option available to you such as:
