Residential Care (RC) Program
CCCHC’s Residential Care (RC) Program provides comprehensive on-site healthcare services to individuals living in residential care facilities who are diagnosed with severe and persistent mental illness. Our goal is to improve access to quality medical and behavioral health services for one of the most vulnerable populations in our communities.
Through this program, our multidisciplinary healthcare team travels directly to residential facilities to deliver care where patients live. The RC team consists of physicians, physician assistants, nurse practitioners, psychiatrists, nurses, medical assistants, and phlebotomists who work together to provide coordinated, patient-centered care.
The Residential Care Program currently serves approximately over 600 residential care facilities. These individuals often face complex medical, mental health, and social challenges that can make accessing traditional healthcare services difficult. Many patients experience conditions such as schizophrenia, bipolar disorder, psychosis, and major depressive disorders, along with chronic medical conditions that require ongoing management.
To ensure continuity of care, patients receive monthly primary care and psychiatry visits, as well as podiatry, Optometry, and Dental services. By providing consistent on-site services, the RC Program helps improve chronic disease management, reduce hospitalizations, and promote overall wellness for residents.
In addition to medical care, the RC Program offers a wide range of support services including phlebotomy, nursing case management, referrals, specialty care coordination and managed care enrollment assistance. These services are designed to address both the medical and social needs of the population we serve.
Many residents in the program face significant health disparities, including limited access to healthcare, higher rates of homelessness, substance use disorders, and histories of incarceration. Through strong partnerships with residential facilities established via Memorandums of Understanding (MOUs), our team is able to bring essential healthcare services directly to patients and provide care in a supportive and familiar environment.
